When you arrive at Terre Haute Regional Hospital, we want your check-in to be as smooth as possible. That's why, whether you are coming as an outpatient or an inpatient, we ask that you proceed directly to our main registration area located at the south end of the main lobby area of the hospital. It is open for registration Mon - Fri: 5:30am - 5:00pm. Outside of this timeframe admissions are handled through the Emergency Room registration area.
You can also preregister online for your scheduled procedure or mammogram.
You may be required to sign the following:
- Consent for treatment
- Authorization to bill insurance
If you are a Medicare recipient, you will be required to sign:
- Medicare Secondary questionnaire
- Medicare Rights Statement
If you are a Champus recipient, you will be required to sign a Champus Rights Statement
